At Open Arms Living LLC, our Supported Community Connection services help individuals actively participate in their communities while building social skills, independence, and confidence. This service focuses on creating meaningful engagement in everyday community life for individuals receiving HCBS supports in Colorado.
We believe community connection should be meaningful, respectful, and person-centered. We focus on supporting individuals in ways that encourage independence while ensuring safety and comfort. We support individuals in exploring their interests, developing friendships, and accessing community activities in safe and structured ways.

What Is Supported Community Connection?
Supported Community Connection is a service designed to help individuals engage in their communities with appropriate support. This includes assistance with participating in recreational, social, volunteer, and skill-building activities that promote independence and personal growth.
The goal is to reduce isolation and increase opportunities for meaningful community involvement.
Services We Provide
- Participation in community recreational activities
- Support with social skill development and peer interaction
- Assistance attending events, classes, or programs
- Exploring hobbies and personal interests in the community
- Volunteer opportunities and structured community involvement
- Support with communication and navigation in public settings
- Encouragement of independence in community settings
All supports are provided based on individual preferences, goals, and approved service plans.
Who Can Receive These Services?
Supported Living Services (SLS) Waiver
Children’s Extensive Support (CES) Waiver
Developmental Disabilities (DD) Waiver (as authorized)
Community First Choice (CFC/IHSS) (if included in approved support services)
Services must be approved through the individual’s case management agency and service plan.
